The following forms need to be completed and filed with our office to create Assumed Business Names.

Please note that the recording fee to file a DBA is $25, pursuant to the standard recording fee schedule.
More formal business types require some filing with the Secretary of State, Corporations Division. It is strongly suggested that individuals consult an attorney before forming a formal business entity. More information can be found in the Indiana Business Owner's Guide.
