Any NEW or REMODELED food establishment in Bloomington, Indiana / Monroe County MUST complete the following Plan Review Application with Questionnaire, then submit the completed forms to the Monroe County Health Department BEFORE construction begins.

Multi-Market Retail Food Establishment License Application: Multi-market Retail Food Establishment means a food establishment that stores, prepares, packages, serves, vends, or otherwise provides food for human consumption at farmers markets.
- Retail Food Establishment Plan Review Application: Retail food establishment means an operation as follows that: Stores, prepares, packages, serves, vends, or otherwise provides food for human consumption.

Farmer's Market Registration Application: (for farmer's market managers only) It is the responsibility of the Market Manager to ensure that food vendors are properly licensed and that exempt food vendors follow state and local guidelines. The Market Manager must register a Farmers’ Market with the Environmental Health – Foods Division at least 30 days prior to the market.
- Temporary Retail Food Establishment License Application: Temporary food establishment means a retail food establishment that operates for a period of no more than fourteen (14) consecutive days in conjunction with a single event or celebration with the approval of the organizers of the event or celebration.
- Pre-packaged Items Only Plan Review Application: Prepackaged Food items only means a retail food establishment that is selling prepackaged foods that remain in the original container with no food prep on site whatsoever, such as a convenience store. An establishment that offers only prepackaged foods that are not potentially hazardous does not require a license from the Monroe County Health Department.
- Remote Catering Only Retail Food Establishment Plan Review Application: Commissary/Catering only kitchens refers to a registered catering establishment, restaurant, or any retail food establishment in which food, food containers, or food supplies are: kept; handled; prepared; packaged; or stored; It may also include an area from which meals are catered and mobile retail food establishments or pushcarts are serviced. For example, meal delivery services, small catering operations, a business provided kitchen space to other retail food establishment operators for rent and/or lease.
- Mobile Food Establishment Plan Review Application: A retail food establishment that is : wheeled; on skids; mounted on a vehicle; a marine vessel; or otherwise readily moveable; such as a pushcart of trailer.
- Seasonal With Permanent Structure Retail Food Establishment License Application: Seasonal food establishment means a retail food establishment that: Stores, prepares, packages, serves, vends, or otherwise provides food for human consumption for 9 months of a licensing calendar year.
